Where this comes from
Dr. Robert Lustig has spent a career as a paediatric endocrinologist, treating children who were sick in ways that were meant to belong to middle age: fatty liver, high blood pressure, the early tracks of type 2 diabetes. The standard instruction was the one everyone knows. Eat less, move more, watch the scale. It did not work, and over the years he came to understand why.
When he took sugar out of his patients' diets, they got better before they lost a gram. Blood pressure eased, triglycerides fell, the liver settled, insulin came down. The weight followed later, if it followed at all. The lesson was hard to miss: weight was never the disease. It was one late signal of something happening much deeper, and you could fix the deeper thing while the scale sat still.
The disease is metabolic
Underneath most modern illness sits a single fault. Sugar and ultra-processed food overload the liver, fat builds up inside it, and insulin climbs and stays high. The body stops listening to that insulin, and once it does, the damage spreads. It shows up as weight, but also as blood pressure, blood sugar, blood fats, and further along the road as heart disease, type 2 diabetes, and more.
This is not a fringe case. On the best measure, fewer than one in eight adults are in good metabolic health. The rest are somewhere on the path without knowing it, because nothing tells them until a number finally crosses a line at a checkup, years after the trouble began.
